Required parent and student workshops will be held on March 18, 2017 and March 25, 2017 9 am - 12:30 pm

PROGRAM REQUIREMENTS

  • Students ages 7 years old through 18 years old (12th grade)
  • Students ages 11 through 18 GPA of 2.0 or higher (students are required to maintain at least a 2.0 GPA for the duration of their membership)
  • One letter of recommendation from an adult teacher, counselor, or athletic coach
  • One letter of recommendation from family member, community, church, or civic leader
  • Parent and students must attend Titus ‘Better Families’ Workshops (no charge for members) (non-members $25 per adult & 10 per child)

      1. Parents and students must attend Titus Scholarship Fundraiser Gala (May 7, 2017 2 pm – 5 pm - Sycamore Ballroom, Lakewood, CA)
      2. Parent must purchase student gala dinner ticket costs $20 for each student and purchase parent gala dinner ticket for $50. Due 4/15/17
      3. Students are required to sell at least 2 gala tickets by 4/15/2017 (Early bird tickets $55 until 4/7/2017, $65 until 5/1/2017)
      4. The more tickets a students sells the more funds he/she has for camp tuition
      5. Parents must complete camp registrations properly and on time.  Titus is not responsible for late registrations which result in loss of scholarship and/or financial aid.
      6. Parents are responsible for transportation to and from camps.  In some cases, camp field trips are the parent’s responsibility (e.g. AC Green Basketball Leadership camp, etc.)
      7. Students are required to complete a 200-word essay on STEAM.  Prompt: Why is STEAM important and how it will benefit me in my future college and career endeavors? (Due by 4/15/2017)
      8. Students must engage in one community service project (4 hours) during February – May 2017 (provide type of project and how it impacted (helped) someone in your community) – signed by community program leader, student, and parent
      9. All registration fees must be paid by April 12, 2017 (Deposit of $35 due on 2/12/2017) Parent + 1 Child ($65), 2nd child $20, 3rd Child $10, 4th Child $5 (1st payment Feb. 12th $35, 2nd Payment March 12, Final Payment April 15). Select the Donate button to pay registration fees.
      10. Agreement that Titus is not liable for and is held harmless from activities that parents and/or my students (see below) engage in with camps and their affiliates.
      11. Agreement to allow Titus to use our photos, without the use of our names for advertising the Titus program
      12. Agreement that Titus may not sell our personal information or use our information for the promotion of other businesses. 
      13. Titus agrees to keep our information private and confidential.
